What treatment looks like
A soft capsule is part of having an implant. Contracture is that scar tightening until the breast is firm, high, distorted, or painful. Grade matters. A breast that looks normal and feels slightly firm is not the same operation as a breast that is hard and misshapen. Infection, bleeding at the original surgery, radiation, and a ruptured implant all raise the chance of a tight capsule.
Before the word contracture is used as the final label, late swelling and a mass are checked so BIA-ALCL is not missed. That may mean imaging and a fluid sample. Once contracture is the diagnosis and it is severe enough to treat, surgery removes or releases the capsule and then decides the breast: another implant in a different plane, your own tissue, fat, or nothing. Replacing a device into the same tight pocket and hoping is not a plan.
Radiation history changes this conversation. Skin that has been radiated often does poorly with another implant. Dr. Sayah will say so at the examination rather than after a failed replacement.
